Is physical therapy the unappreciated child of healthcare? The talk with Heather Linden from the UFC

How does physical therapy help athletes like Ronda Rousey, George Saint-Pierre or Conor McGregor? Why is it important to involve physiotherapists in the training of athletes before a fight and after the fight as well?

Heather is the Director of Physical Therapy Center of the UFC. The UFC managed to establish mix martial arts as a respected and highly valued sports discipline gaining popularity across the globe. It is only natural that they would set the standards of care regarding physical functionality for athletes in this sport. Heather explains how she transitioned from olympic athletes program to the UFC. The beginnings were not easy, as many athletes were not familiar with a physical therapist, yet her and her team managed to gain the trust they needed to increase the performance of the athletes. Heather shares her views on the "hot vs cold" controversy and clarifies how electro-stimulation can be used. In her opinion, a lot of doctors don't know enough about the level of professionalism in physical therapy and neglect to cooperate and communicate more with experts in this field, often with severe consequences.

How quickly can the modern medicine detect bacteria? Why is the communication between bacterial colonies so important to understand?

Fatima approached a bio-chemical phenomenon in her own way, her background as a physicist, her drive and perseverance made her discoveries worthwhile.  She developed a sensor that makes it possible to detect bacteria in any sample within 60 seconds, even in early stages, where other more commonly known methods would not show the results. This is a ground breaking thing for patients, who depend on the early recognition of infections, since their immune systems might be deficient. Find out more about her journey in the interview.

What your doctor doesn't want you to know? The talk with Cathrine DesRoches, PhD

How much more transparency is required for medicine to truly help the patient? Are doctors afraid of losing their privileges? 

Cait and I dive into the pros and cons of OpenNotes. Imagine that as a patient you could access the transcripts from all your clinically relevant moments with a physician or therapist freely and at any moment. It is possible that patients would engage much more with their health, not only prepare for next visit at the doctors office but also double check on their previous conversation with the clinical staff. In Cait's experience, working with over 200 health institutions in the US, OpenNotes brought a lot of benefits on both sides. Meanwhile, some people remain sceptical about sharing patient's notes, afraid of increasing the administrative efforts or dealing with obnoxious patients. Time and practice will show, if complete transparency is the right way, how to change patients' engagement with health services for the best.

Wie wird heute die Makuladegeneration behandelt? Warum sollten sich mehr Ärzte als Unternehmer versuchen? Diese und weitere Fragen werden in dem angenehmen Talk beantwortet. 

Wirklich ein breites Themenfeld zeichnet diesmal das angenehme Gespräch mit Robert, der als Facharzt für Ophthalmologie eine Marktlücke entdeckt hat. Oder sollte man es lieber Verbesserungspotenzial nennen? Jedenfalls ist die Therapie der Makuladegeneration eine aufwendige Sache, da man alles sehr gründlich sterilisieren muss. Genau da setzte Robert mit seinem Team an und versuchte mittels 3-D Printing einen Prototyp herzustellen, arbeitete dabei auch mit Feedback von den Fachkollegen und Patienten.  Es ist nicht einfach ein Produkt herzustellen aber die danach folgenden Schritte der Massenproduktion, Vermarktung und Etablierung sind ja auch nicht ohne Tücken.  Es ist dennoch erfreulich zu sehen, wie junge passionierte Menschen die Medizin im Hintergrund vorantreiben.

"Anaesthesiologists should use EEG monitoring during general anaesthesia!" The talk with Prof. Brown

Can computational sciences help anaesthesiologists in their daily work? How much do we understand general anaesthesia and the effects it has on the brain? Do we really need to accept that we simply feel groggy after waking up from general anaesthesia?

As Professor brown explains, the general idea about anaesthesiology is, that nobody knows how it works, although it is done daily, in the US alone around 60 000 times. By employing EEG it is possible for Prof. Brown and his colleagues to determine very precisely how specific drugs effect specific parts of the brain, showing unique patterns in the EEG. Common misconception about their complexity is refuted by Prof. Brown, explaining that reading an EEG is not much more complex than say reading a ECG. He also explains that the choice between general anaesthesia and local one is based on medically relevant factors, not on the wish of the patients. He also gives a brief introduction in his early research on the circadian clock and its changes due to light adjustments. Of course I had to ask about those scary cases when people felt their surgeries since they were not in a induced reversible coma, but they could not move. Prof. Brown shed some light on these cases.

Don't' take drugs, but if you do, check what's inside first! The talk with Daan van der Gouwe

Maybe you are against drugs, maybe you are for some kind of middle ground regulation, or maybe, you are a hippie! It doesn't matter, because what is important is that if a drug user decided to take drugs, the best we can do is educate them about the substance and make sure the purity of the substance is high!

Daan is in the harm reduction and drug control for a lot of years and what he saw during those years shaped his understanding of what's going on for the better. Besides having a unique insight into the classic on-the-street drug market, he knows about dark/deep net market places. The stations that his colleagues work at provide the initial possibility for drug users to ask for help or guidance if they need to do so. Additionally, by checking the drug purity, Daan and his colleagues already saved thousands of users from potential fatal consequences due to impure drugs available on the market.

Why does anybody need a personal trainer? Why do some people have such a problem starting to engage in physical exercise and can the wrong motivational base for training be harmful?

Nino is a professional personal trainer from Georgia and she explains how the differences in the educational and cultural framework between Georgia and the UK account for the number of people exercising in both countries. She discusses with me why some people won't be happy with their bodies, even if they engage in physical activities and she highlights the fact that personal trainers have to take care of the mental side in their clients just as much as they care for the physical stuff. We both acknowledge the dangers of social media sport gurus/influencers and break down, why it isn't absolutely wise to try replicating what you see online. As a bonus Nino explains why the controversy on static and dynamic stretching doesn't make much sense.

Trauma erlebt und noch nicht mal auf der Welt? Mir ist es auch passiert und ich habs ja überlebt also was soll's? 

Das Thema Psychotrauma hat Herrn Professor Ruppert seit langen Jahren beschäftigt. In seiner täglichen Praxis begegnet er Menschen, die meist als Kinder Situationen erlebt haben, die ihre Wahrnehmung, ihr Selbst komplett überfordert haben und die Psyche reagierte darauf mit einer eigenartigen Strategie. Die Belastung wurde abgekapselt und in den Keller geschoben, dort liegt diese tickende Zeitbombe meist auch bis ein ähnliches Ereignis die Bombe aktiviert. Es wird auch klar, dass die Frage nach dem "Wer bin ich?" oft mit einer Identifikationsantwort befriedigt wird. Dies hat auch ernste Folgen für das einzelne Individuum. Sie erklärt eindrücklich wie die Zahnabnutzung immer mehr zunimmt und dies auch psychisch bedingt ist. Als sensibilisierte Zahnärztin auf das Thema Angst bei Patienten, arbeitet sie interdisziplinäre und nutzt Methoden wie die Hypnose. Ich habe erfahren dürfen, warum die Zahnbehandlung in der Schweiz teuerer ist als im Ausland und Julia gab auch ihre Meinung zum Thema social Media preis.

Ist Anatomie wirklich so schwer zu lernen? Warum ist ein gewöhnlicher Medizinstudent mit diesem Fach überfordert? Was ist KENHUB und wie hilft es den Studenten genau?

Diese Fragen werden von Christopher innerhalb unseres lockeren Gesprächs beantwortet. Er macht es deutlich, dass man langsam aber sicher nicht nur die Inhalte sondern auch die Art und Weise des Medizinstudiums verändern muss. Schliesslich stehen uns heute sehr viele Mittel zur Verfügung, die man dafür nutzen kann. Er erklärt wie wichtig es ist, das richtige Team dabei zu haben, wenn man eine geniale Idee auch umsetzen kann, sonst bleibt es nur beim Träumen. Und Schliesslich verrät er uns, wie das Leben eines Assistenzarztes zumindest in seiner Klinik aussieht. Eins wird klar, die Strukturen haben sich seit einigen Jahren vermenschlicht und die Kommunikation zwischen den Ärzten egal auf welcher Stufe ist das A und O.

"Fly like a butterfly sting like a bee!" The talk with a ringside doctor Davide Bianchi, MD

Did you know that when a fighter gets cut, it is not the ringside physician who treats him first? Who can or should become ringside physician and why is it essential to have one on the spot?

In this informative and lively talk, Davide explains to me what it takes to be a ringside physician. As a fighter he understand, what nuances must be taken into account before heroically stoping the fight and maybe robbing someone of fame and money. As a ringside physician he understands the required calmness in heated situation, when audience is not happy. Damage to health in combat sports however does not only occur during a fight, but also during the preparation phase. Bad training methods and dehydration to make weight are the killers before any fight. "Fighters can water up in 24 hours but there is no way their electrolytes can be restored as quickly." This results in fatigue and consequences during the fight. Listen to Davide and his anecdote and you will know why!

Was wissen wir über die kleinen Weltentdecker? Der Talk mit Prof. Moritz Daum

Wann erlernen Kinder die Sprache und wieso gibt es da individuelle Unterschiede? Wie können den Eltern die Kinder optimal in ihrer Entwicklung unterstützen?

Auf diese und viele weitere Fragen antwortet Professor Daum von der Universität Zürich auf eine spannende und vor allem klare Art. Er schildert, wie wichtig die Teilhabe an der Entwicklung der Kinder seitens der Eltern ist. Die Zone der optimalen Entwicklung befindet sich ja bei Kindern dort, woe sie zwar Schwierigkeiten haben aber diese selber bewältigen können. Nicht umsonst sprachen wir über das Kind als "Schwamm", den Kinder müssen genügend Input haben um sich weiter zu entwicklen. Eltern sollten nicht zu sehr an Ratgebern hängen und sich eher durch die gemeinsam verbrachte Zeit sensibel an das Kind anpassen und es unterstützen. Die App, die er mit Kollegen entwickelte und die Eltern helfen soll, "Die kleine Weltentdecker App" wurde im Gespräch ebenso vorgestellt. Auch vor kritischen Themen haben wir nicht gezögert und die Frage über vegane Ernährung bei Kindern aufgeworfen. Ausserdem thematisierten wir gemeinsam die digitale Flut und wie das die Kinder wohl beeinflussen wird. Und zuletzt erörterte Professor Daum die Gender neutrale Erziehung.

When healthcare doesn't care! What can palliative care do for the terminally ill? The talk with Timothy Ihrig, MD

What ist the question that medicine needs to answer for itself in the next few years? Why do we need to rethink the way healthcare is applied?

Imagine somebody you love and adore is terminally ill, now imagine that medicine does things that person and not for the person! How would you feel? Tim and I tried to get to the bitter truth of dying and topics connected to death in the reals of healthcare. We touch on the topic of physician's responsibility, where Tim explains the necessity for truth over fear. He explains that changing the narrative from dying to "last period of living" empowers the patient to set a priority on "what is sacred to them". We discuss the assisted suicide and the choice of pain management in general. Overall the honest and direct communication with the patient and his family seems to be absolutely crucial. Tim offers statistical evidence for the efficiency of true care and helps us understand how economically futile the obsessive "let's try and fix it" ideology can be.
