Medical Forum’s Clinical Director, Dr Joe Kosterich, caught up with clinical cardiologist Professor David Playford to discuss the prevalence, prevention, and treatment of Australian heart valve disease – a leading cause of mortality.
“Heart valve disease can mean any valve disease, including aortic stenosis, aortic regurgitation, mitral stenosis, mitral regurgitation, tricuspid valve disease and pulmonary valve disease,” Prof Playford highlighted.
“There is no single symptom that is unique to any particular heart disease, including heart valve diseases, but the most common symptom is breathlessness. The most important point though, is that most heart valve disease is asymptomatic particularly in the early stages, and only becomes symptomatic late in the course of the disease. We don’t want to wait until symptoms occur before we identify heart valve diseases.”
“Early diagnosis using echocardiography is critical to prevent heart failure or potential death, and so we recommend regular auscultation in older Australians at least once a year. Our research found that most patients have surprisingly low understanding of heart valve disease and don’t see it as a health concern. This means that GPs need to have a high index of suspicion, listen to the heart regularly, and refer patients with a murmur for investigation.”

Medical Forum’s Clinical Editor, Dr Joe Kosterich, met with spinal surgeon Dr Paul Taylor from the Neurospine Institute to discuss the impact and prevalence of back pain in Australia.
“Back pain is very common - almost everybody at some point in their life will have some low back pain - something like 97% of humanity experiences back pain at some point in their life. And it's the greatest source of lost time to work of any other illnesses and injuries,” Dr Taylor said.
“And having been a patient with a lumbar spine, disc prolapse, and a cervical spine disc prolapse, I can tell you that neural compression pain is horrible. Regardless of what medication you take, you just can't get away from it. It feels terribly dramatic for the patient, and understandably so.
“But there is a bit of difference between a bit of tingling and numbness in a foot or leg, to having a foot that doesn't work. And after a couple of years of running NSI, we realised that several acute disc prolapses, typically in the neck and lumbar spine, were still only being seen in the usual course of referrals, even in private.”

Medical Forum's Clinical Editor, Dr Joe Kosterich, talks with Perth radiologist Dr Conor Murray, from ChestRad, about the latest and upcoming developments in Australian lung cancer screening.
“Lung cancer is the biggest cancer killer of both men and women, and I think that's often a surprising fact,” Dr Murray said.
“All efforts so far, beyond cessation of smoking, have had a trivial effect. The interest in screening for lung cancer with low dose chest CT arose about 20 years ago and the theory or hypothesis was that if you could pick lung cancer up at an earlier stage, then you can extend lives and improve the quality of life.”
