👾 Physostigmine is a reversal agent for anticholinergic toxicities. By inhibiting acetylcholinesterase, increased acetylcholine becomes available at the synapses. It is an excellent drug to reverse antimuscurinic poisonings. Unfortuanetely, it has a negative stigma from prior case reports associated with TCA deaths. The Akorn manufacturer went bankrupt recently, leaving the US without product available for the market. Recently, the FDA announced they would allow temporary importation from a company in Germany. Induction agents are an essential component of rapid sequence intubation. They work synergistically with neuromuscular blockers. As the paralytics relax the vocal cords for optimal conditions as the endotracheal tubes is being passed, induction agents provide amnesia and make the patient unconscious. That is an important thing to mention. How much drug does the patient really need when they're body is barely hanging on? There is such a thing as drug toxicities and we want to keep it safe. The original use of LOAD has fallen out practice for severe TBIs, but yet, the acronym is still discussed/taught as a part of pretreatment. Out of lidocaine, opioids, atropine, and defasciculation dosing, opioids are the only intervention with consistent data on neuroprotective properties in the setting of RSI and TBI. I have modified the acronym "LOAD" to lidocaine, opioids, access, and down or delay. Consider these questions for each: Is this an indication for topical or nebulized lidocaine for an awake intubation?
Consider applying it to your practice if you find value in going through pretreatment LOAD this way.
