Who is That in the Mirror? Self-awareness Affects How We See Our Own Faces

Did you know that the way you see yourself in the mirror is different from the self in photos? This episode touches on fascinating research on the different pathways involved in what scientists call the mirrored versus the non-mirrored self.

Ever wondered what’s actually going on in your mind when you look at yourself in the mirror? According to science, our faces in the mirror versus our faces seen on photographs of ourselves are supposed to be nearly identical. Yet, the truth is we know we somehow look different. Why is that so? The rise of mental health disorders related to body-image has been linked to social media use, in particular following and engaging with celebrity accounts. Being excessively concerned about how we are portrayed on social media, the overvaluation of likes and comments also puts us at risk. But there is a deeper root to the problem—what actually happens when we take on our digital identities? It all boils down to thinking of what others are thinking of us, and that proves to be a psychologically risky path.

In a 2008 paper, Self-awareness affects vision, Eric et al. wanted to determine if high level mental factors such as states of self-awareness can influence perception— and it turns out that our consciousness can be manipulated to cause us to “see” our faces differently. More specifically, in this experiment he wanted to see if there was a difference between how we see the mirrored self, versus the one in a photograph for example. In essence, what he found was that when we are looking at ourselves in the mirror, our awareness is directed internally, the same for instance when we are told to focus on our own breathing, which was what he asked the participants to do. On the other hand, when we look at our faces on photographs our awareness is directed externally. Now what exactly does that mean?

An example is when we are told to think about our strengths and weaknesses. When we do so we are actually thinking of ourselves through the eyes of others i.e. what are others thinking when they look at me? So the researchers actually found the fascinating shift in our self-awareness when we are looking at images of our faces, which change from internally-directed self-awareness in the case of our faces in the mirror, to externally, socially directed self-awareness. Now what do you think are the implications of this? The experiment was performed way before the advent of social media—Instagram was launched in 2010 but really took off about 2014 or so.

The digital self on social media is the epitome of the image that is primarily perceived by an externally-directed self-awareness. And that of course raises questions as to what happens to our psyches when we become obsessed instead with what others think of us?

Instagram has been linked to body-image related mental health burden like anxiety, depression and body dysmorphic disorder. But what research really shows is that it is not the platform itself but the following of celebrity accounts, engaging with them and excessively curating photos that puts one at risk. Also, back in 2008, smartphones were still not invented, and selfies were not possible the way it is now. Do you think the brain perceives selfies via internally-directed self-awareness, like one looking into a mirror, or via externally directed self-awareness i.e. thinking of what others think of you? Let me know in the comments.

Ask a Dermatologist: The Secret to the Perfect Medifacial

Bonus Flash Tutorial

The rise of aesthetic practice in Asia was a phenomenon fuelled by K-beauty. Korean celebrities with impeccable glass skin and even every day men and women on the streets became the envy of all their Asian counterparts. What was their secret?

Well, there is indeed a science-backed reason for this phenomenon. Dermatology clinics were highly accessible in South Korea, and professional skincare was the norm for the everyday South Korean. As a dermatologist, I’m going to spill the beans on how the medifacial experience can be exactly replicated in the comfort of your own home, the complete home facial steps at less than a fraction of the cost, thanks to the advances in home beauty devices.

There are a few types of clinical grade facials you can “hack”. Get the full workshop complete with lecture notes https://www.twlskin.com/home-facial-steps-dermatologist-korean-medifacial/

Face Serum Recommendations by a Dermatologist: How to Choose Face Serums for Your Skin Type

In today’s episode, Board-Certified Dermatologist Dr. Teo Wan Lin teaches you how to choose the ideal face serum for your skin type. Face serums are essentially lightweight formulations that contain water-soluble active ingredients like glycerin, hyaluronic acid and polyglutamic acid instead of ceramides or lipids which are typically present in cream formulations. What are the advantages of using a face serum in your skincare routine and what is the ideal face serum? Listen on as Dermatologist Dr.TWL explains these face serum recommendations in this video.

Let’s go through the actual paper to better consider the validity of viewpoints that have emerged, namely that the extreme temperatures involved in benzene byproduct generation are unrealistic scenarios.

On-Demand Workshop on Skincare Routine Steps

Welcome to The Slide Show with Dr.TWL Episode 2, where we present skin science in bite-sized formats. Today we will be covering the first, fundamental step in skincare routines, makeup removal.

Makeup removal

What are the two main formulation options for makeup removers?
The first is a micellar formulation. The second, an oil-based cleanser or an emulsion, sometimes known as a “milk” cleanser. I will go through the pros and cons of each but first let us define some terms.

How does micellar water work?

Micellar water works by hydrophobic (water-hating) and hydrophilic (water-loving) properties of a micelle. The makeup residue is attracted to the water-hating aspect, this is best thought of as a ball that is wrapped up which continues to roll on skin grabbing the residual pigment and dirt. To remove the ball which is a micelle, you use a cotton pad. Friction or rubbing is inevitable and this can be harsh for sensitive skin.

What is the downside to oil cleansers?

Pure oil cleansers are often too greasy, my choice is an emulsion or a milk cleanser. Milk cleansers are oil in water formulations, the oil component dissolves makeup pigments and removes excess sebum. The “like for like” principle here is that oil soluble pigments are dissolved in a similar substance—an oil in water emulsion.

What is the difference between emulsion vs oil cleansers?

The benefits of an emulsion are that it is less greasy than pure oil formulations. It is also effective as a humectant if formulated with moisturising ingredients. When you physically rub off the makeup on a cotton pad, the oil component protects your skin. It acts as a barrier between the cotton pad and skin, hence reducing friction unlike with micellar solution.

Ask a Dermatologist: Science of Skin Cleansing

How does cleansing affect the skin microbiome?

Cleansing is an important part of the skin microbiome because it helps to restore the natural balance of good and bad bacteria. In acne prone individuals, we consider propionibacterium acnes as a pathogenic bacteria. P acnes otherwise doesn’t cause problems in other individuals. The reason is because excessive growth of p acnes occurs in individuals who produce a certain type of oil- defined by its physical and chemical composition. P. acnes has been identified in acne prone individuals which drives a vicious cycle of inflammation and comedone formation.

Facial Mists for Skin Barrier Repair

Our skin’s source of water comes from the deeper layers of the skin. When the skin is initially exposed to a dry environment, our skin reacts by delivering more water to the skin surface. The surge of water supply eventually plateaus (given there is only so much that the deeper skin layers can delivery), and this ultimately leads to dry skin.

A facial mist delivers water content to the skin surface to preserve hydration levels. While some of the facial mist will be lost to the surroundings, it still helps to keep the skin moisturized.

A mist also enhances skin viscoelasticity by softening the skin layer and maintaining skin moisturization. You can expect greater elasticity or resilience with frequent spritzing of your favourite mist.

At the Heart of the Skin Barrier: Water

Skin Anatomy & Physiology at the Heart of Barrier Function & Hydration

You may already be aware of the importance of skin barrier function—how it can affect the condition of the skin, but how exactly?

Skin hydration and the stratum corneum barrier have been active areas of dermatologic research for a good decade. Alongside these developments, consumers are beginning to find their interest piqued about the subject, largely because of the marketing campaigns by industry giants.

Before you commit to any product or treatment that promises to ‘repair’ the skin barrier, I’ll teach you exactly how to navigate industry jargon so you don't get confused, worse still, foxed by the deluge of claims—beginning with the basic science of skin.

Dior Prestige Le Nectar Premier: The First Rose, Pure Like First Love

The Dior Prestige Le Nectar Premier encapsulates the earliest blooms of the beloved Rose de Granville—a serum derived from patented solvent-free eco-extraction techniques. Backed by an international team of over 600 Dior Science researchers at the LVMH institute, the serum concentrates the purest extracts for maximal efficacy—targeting fine lines, volume loss and skin dullness in a single step, addressing all major signs of photoaging with a holistic botanical formula.

This week’s episode focuses on the science behind photoaging.  I’ll also address how botanical formulations can address underlying inflammaging processes holistically, reducing signs of aging such as dull skin, hyperpigmentation and irregular texture.

Let’s begin with what dermatologists term the skin exposome concept. The exposome refers to both environmental and biological processes that influence cell aging. Throughout our lifespan, cells work constantly to repair damage sustained day to day, UV radiation, airborne pollution and exposure to carcinogens result in mutations. The body’s natural physiological processes actively target these abnormal cells, engulfing them before they have the chance to turn into cancerous growths—a process known as apoptosis.

However as one grows older, this balance of protective mechanisms vs environmental damage is lost—cells become fatigued, resulting in what is known as cell senescence. A gradual falling asleep that ultimately leads to an inability to clear out abnormal cells. This is what eventually causes hyperpigmentation and dullness. On the top-most layer of skin, also known as the stratum corneum, dead skin cells begin to accumulate as the skin cycle lengthens. Skin shedding is the body’s natural exfoliation mechanism which allows new cells to regenerate, revealing radiant, healthy skin. The loss of cell energy within the mitochondria also means that the body will be unable to clear excess melanin pigment, resulting in accumulation and the development of hyperpigmentation. Collagen production also slows down, leading to skin sagging and wrinkle formation.

Dior’s Rosapeptide Premier formulation contains 99 molecules that targets the 3 critical signs of aging—volume, texture and radiance. At the heart of which is the phytochemical-rich composition derived from the earliest Rose de Granville buds. Picked precisely in the week before full bloom, the buds undergo solvent-free extraction processes that utilise water innate to the blooms—combined with Rosapeptide, a 99 molecule-rich extract that results in perfect synergy, a protective anti-oxidant, anti-inflammatory and nourishing concentrate that targets all major signs of skin aging, revealing smooth, radiant, plump and elastic skin from the inside out.

The Dior Prestige collection is a complete range featuring precious Rosapeptide, derived from Rose de Granville—the first rose from Monsieur Christian Dior’s garden, the heart of beauty. The Slide Show Ep 1: Skin Barrier Workshop

The Slide Show is a series of mini workshops designed for learning on the go, with easily digestible bullet point slides and helpful visual aides. You'll get my pdf notes sent to your inbox plus an accompanying lecture. In this week's installment, we'll be covering the essentials of skin barrier function—must know skin science for skin experts.

Imagine the skin like a brick wall that serves to protect your skin from the external environment. It keeps out allergens like dust, pollutants and bad germs. At the same time, it also adapts to the environment. Healthy skin adjusts to changes in temperature, humidity and is also adversely affected by disturbances in the microbiome. What you need to understand is that it is ultimately the balance in this ecological system that matters—not precise values. This is why topical antibiotic treatments for various dermatological conditions have failed—acne skin is affected by Cutibacterium acnes, but it isn’t the bacteria alone that’s causing the disease. In this tutorial, I'll take you through the anatomy and physiology of the skin barrier—the foundations of essential dermatology for skin experts.

A Conundrum: The Science Behind Hyperpigmentation & Sensitive Skin

Hyperpigmentation is a key feature of photoaging, along with the development of wrinkles, loss of skin elasticity and facial volume. Pigmentation isn’t just a cosmetic concern—it can be a sign of underlying inflammation. Post-inflammatory hyperpigmentation for instance arises from conditions such as eczema or other forms of dermatitis, all of which are part of what is widely referred to as sensitive or reactive skin. Acne scars are also a form of hyperpigmentation, beginning with post-inflammatory erythema or redness immediately after acne resolves. This redness is a sign of inflammation, which later becomes pigmentation as the melanocytes begin to produce melanin. Melanin deposited in the superficial and deeper layers become what we know as hyperpigmentation.

Traditionally prescribed tyrosinase inhibitors such as hydroquinone reverse this process by blocking the melanin synthesis pathway. However, hydroquinone is also associated with skin irritation which can occur even with short term use. Sensitive skin for example does not respond well to hydroquinone therapy—it is possible for flare ups to occur necessitating the use of topical steroid therapy. For instance the 3-in-1 hydroquinone-retinoid-steroid formula known by its trade name Triluma was specifically created to counteract these side effects during the treatment of melasma. Such an approach isn’t a solution either—those with sensitive skin are susceptible to side effects of irritation, caused by barrier disruption.

Is it possible to treat hyperpigmentation without the use of prescription topicals? Hyperpigmentation treatment in sensitive skin types deserves a more in depth study— join me as I dive right into the science of this conundrum. I’ll share the latest research on bacterial ferment filtrates as melanogenesis inhibitors, alternative tyrosinase inhibitors and the exciting field of functional dermatology.
