Treating Misdiagnosis - The Future of Patient Safety

In this episode of the SMART Healthcare Podcast, Matthew interviews Dr. Mark Graber, founder of the Society to Improve Diagnosis in Medicine (SIDM). Diagnosis is no easy feat, and it requires the physician to carefully consider a multitude of factors through a process known as a differential diagnosis; in 90% of the cases, the correct diagnosis is able to be reached via this method. However, in the other 10% of cases, something goes awry, resulting in a false diagnosis. The errors that result in misdiagnosis are quite broad, ranging from human error to systemic problems affecting how fast lab results or other information is made available to the physician. A big part in mitigating these errors has been the rise of the Electronic Medical Record (EMR), which has made information more accessible to the physician, while also allowing the patient to begin playing more of a role in diagnosis. In tandem with the EMR, the creation of clinical decision support (CDS) software is helping provide physicians with even more context in regards to what conditions they should be considering when creating a differential diagnosis.
