Almost half presented with dysplastic lesions, mostly low-grade, with only a small percentage progressing to more serious conditions. High-risk HPV genotypes, detectable HIV viral load, and initial symptoms were identified as independent risk factors for advanced precancerous lesions. While no patients progressed to invasive cancer during follow-up, the findings highlight the importance of regular screening and early intervention, particularly in high-risk groups, to prevent anal cancer progression. The study also resulted in a refined institutional screening algorithm.
