由 2025 年 7 月 1 日起，澳洲的國民保健福利計劃 (Medicare Benefits Schedule，簡稱 MBS) 將在慢性病管理方面作出更改，包括以單一的「全科醫生慢性疾病管理計劃」取代現有的「全科醫生管理計劃」和「團隊護理安排」。
